What is a Somatics GP assistant?

​

Your GP works with a somatics practice assistant (abbreviated: “POH S”) to treat and guide people with chronic conditions such as:

  • diabetes;

  • cardiovascular disease (or a risk thereof);

  • lung diseases (asthma and COPD);

  • guiding vulnerable elderly people and those who have just been released from the hospital.

The POH S always works under the responsibility and guidance of your GP. The national guidelines of the Dutch College of General Practitioners and Zorggroep Medrie are leading in this.

What can the POH do for you?

​

With chronic conditions, it is important to have regular check-ups to treat you well and prevent complications. Most of these checks are done by the POH. She also provides information about the disease and how to deal with it. The POH has several consultation hours.


Diabetes office hours

​

Checks of patients with diabetes take place periodically. In addition to discussing your blood test and physical examination, attention is also paid to your lifestyle (diet, exercise), the use of medication and the consequences of diabetes in your daily life.


Cardiovascular disease consultation hour

​

Regular check-ups are important for people who are at increased risk of developing cardiovascular disease or who have had cardiovascular disease. The POH checks blood pressure, weight and other risk factors, among other things. She will also discuss the results of your blood test, which have been assessed by your GP. If necessary, she will make a plan with you to change your lifestyle and use medication.

Asthma / COPD consultation hour

​

Regular monitoring and measurement of lung function is important for people with asthma or COPD to keep the condition of the lungs optimal. The POH S can help you with questions about your lungs, the medicines you use and how to deal with your complaints during your everyday activities.

Quit smoking

​

If you want to quit smoking, you can also discuss this with the POH S or your doctor. Our POH S are trained to guide people to quit smoking. Quitting smoking with support appears to be more effective than quitting independently. Feel free to contact us if you are interested.

How does it work?

​

The GP will refer you to the consultation hour of the POH S. You can also receive a call, because according to our information you qualify for extra guidance. Of course you can also contact us if you want to visit the office hours of the POH S.

​

What are the costs?

​

There are no costs associated with the consultations at the POH. These are fully reimbursed by your health insurance.

​

What if I am unable to attend?

​

If you are unable to attend, please cancel the appointment in time. In the time that was reserved for you, someone else can be helped. If you do not cancel in time, the costs may be charged to you. These costs cannot be recovered from your health insurance.
